What travel documents are needed to visit Tibet?

I am planning a trip to Tibet this June. I have heard that foreigners need some documents to visit Tibet. Please tell me what travel documents are needed to visit Tibet?

Travel Permit as well as a valid passport and a visa (Travelers from countries having visa exemption agreement with Hong Kong do not need a visa) are needed to visit Tibet. A visa can be obtained from the local Chinese consulate in your country.
